Improving Outcomes Through Planning
TRANSITION PLANNING prepares students to move from secondary settings (e.g. middle school/high school) to adult life utilizing comprehensive transition planning, strategies, and evidence-based lesson plans that create opportunities, services and supports that help students achieve their college, career and community goals. This website provides the tools for Individual educators and other school teams to assess their local and county Post School Outcomes (PSO), and use that information to create a transition improvement plan based on the National Predictor of Postschool Success that positively impacts transition goals and student outcomes. In addition, individuals and teams can learn about the National Dropout Prevention strategies that will positively impact successful school completion and graduation rates.
