Wisconsin Transition Improvement Plan
 

TRANSITION PLANNING prepares students to move from secondary settings to adult life by utilizing comprehensive transition planning, innovations, and evidence-based lesson plans to create opportunities, services and supports that help students achieve their college, career and community goals. This website provides tools for individual educators and district or other school teams to create a Transition Improvement plan (TIP) by (1) reviewing state and local post school outcomes, (2) assessing the extent to which students have exposure to the "In-School Predictors of Post School Success ", (3) reviewing high quality resources, evidence-based practices and lesson plans, and (4) selecting "high priority" practices to implement through a transition improvement plan. These efforts will positively impact the post school outcomes of district youth.
